Parents who would like to book an appointment for their child to attend a community immunisations clinic can do so by using the bookable URL. This will open the bookable eConsent form.
The first section of the form is the Consent Check: the parent will be asked to provide the child’s first name, last name, date of birth and postcode to check if there is already a consent for that child.
RIVIAM will only allow parents to continue to book an appointment if certain criteria are met.
If RIVIAM completes the search for the child's consent and returns the following messages, there are specific reasons:
- "We could not find your child's details; please check the details or click 'Continue' to complete a new consent and book an appointment" The system didn't find any records matching the demographics submitted for that specific immunisation programme; therefore, the parent will need to complete a consent form and make a booking.
- "We are unable to proceed with this booking. Please contact XXX on XXXX or email us using xxx@xxx.com": This message appears if the child has either been vaccinated already or has already made a booking.
- "We found your child's consent details. Please click Continue to book an appointment." This means that consent has already been submitted for that child in that specific programme, and the parent can continue to book an appointment.
Please note: If no existing consent has been found, the parent will be asked for details about the school the child attends. If the parent selects no because the child is homeschooled or does not attend a school setting, then the new consent submitted will be included in the community clinic list and reported as being outcomed in the community clinic as opposed to a school. |
Once the school is selected and the person clicks continue they will be asked to complete the remaining student details, parent/ guardian details, consent, and history.
After this information is completed, parents will be asked to choose a venue, date and time to make a booking and then confirm their request.
They can also choose to receive an SMS confirming their appointment plus a reminder SMS text 24 hours or 48 hours before the appointment time.
For new consents submitted, the parent will receive a consent confirmation email detailing the date and time of their appointment.
