I'm filling out a survey, but there's an error preventing me from submitting the form
If you have been invited to complete a questionnaire or survey, you will receive an invitation link via email or SMS.
When you open the link to verify your identity, you will be asked to enter your date of birth as recorded on the original referral. This referral may have been made by you (self-referral) or by a professional. If the date of birth does not match our records, you will see an error message:
After this verification procedure, you can begin filling in the survey. Each question contains mandatory fields, so please ensure you answer all of them.
In the 'About you' section, please make sure to enter the same postcode used in your original referral. Avoid adding any spaces before or after the postcode, as this may trigger an error message.
At the end of the survey, you will be asked to check your answers before submitting. Once submitted, you will see a success message similar to this:
